Advanced Care Planning for Last Days of Life
At The Manor Nursing & Residential Care Home, we bring extensive experience in delivering exceptional End-of-Life (EOL) and Palliative Care in Plymouth. We are inspired by Dame Cicely Saunders’ ethos and believe it is vital that when our residents pass away, they are comfortable and, in a setting, where they feel at home and at peace.
Advanced Care Planning (ACP) discussions are essential when someone is nearing the end of their life. These conversations will be led by our Registered Manager, Shannon Young. She will speak with the resident and their loved ones to understand their specific wishes and to foster a supportive and peaceful environment during their final days.
St Luke’s Six Steps End of Life Champions
To ensure we provide the best possible care during an individual’s last days of life, we have an End of Life Champion Registered Nurse, along with another registered nurse currently undertaking the 6 Steps End of Life Champion Course with St Luke’s Hospice Plymouth.
St Luke’s offers end-of-life care and support in the community and at its hospice care facility. The Six Steps + Programme, created by St Luke’s education team, consists of workshops designed as a toolkit for Care Homes and agencies that provide end-of-life and palliative care, supporting adherence to best practices and CQC (Care Quality Commission) standards. As EOL Care Champions, we focus on sharing our knowledge and skills with peers and colleagues.
